Documents Required to Apply for Membership
- Completed two page Application Form
- Original Transcripts
- Verification of Practicum/Internship Hours (for applicants who graduated after May 1, 2011) - Download Practicum/Internship Reporting Form
- Completed first page of Application Form
- Photocopy of Student ID
The Membership Review Committee approves all Professional Memberships. The Committee meets once per month to review all completed applications. Incomplete applications will not be reviewed until all of the required documents and fees are received (e.g., original transcripts). Once membership is approved, it will generally be processed within two weeks of the committee meeting. If there are any issues with the membership, the Membership Review Committee will contact the applicant for further information.
